Last updated: to complete before going live.

To personalize: This template covers standard data-protection notices for a contact form. Adapt the fields marked "to complete" and have it reviewed by counsel before you begin collecting assessment data (Level 1/2 questionnaires), which involves broader processing than a simple contact form.

1. Data controller

The data controller for information collected via this site is [Legal company name], [legal form], registered office at [address], registration number [number]. Contact: contact@homo-digitalis.fr.

2. Data collected

The contact form collects: full name, work email, institution, role, and the content of your message. No payment data or sensitive personal data is collected through this form.

3. Legal basis

Processing is based on your consent, given explicitly via the form checkbox, and on the controller's legitimate interest in responding to inbound requests.

4. Retention

Contact data is kept only as long as needed to handle your request, then deleted within [period to complete, e.g. 12 months] if no further engagement follows. Technical server logs are kept for a maximum of one year.

5. Recipients

Data is used exclusively by the controller's team. It is never sold, rented, or shared with third parties for marketing purposes. The hosting provider (see legal notice) may technically access data as part of its hosting role.

6. Your rights

Under the GDPR, you have rights of access, rectification, erasure, restriction, objection and portability. Exercise these by writing to contact@homo-digitalis.fr. You may also lodge a complaint with your local data protection authority (in France, the CNIL — www.cnil.fr).

7. Cookies

This site does not use audience-measurement or advertising cookies. A single technical session cookie, strictly necessary to protect the contact form against fraudulent submissions, is set and does not require prior consent.

8. Security

The site is served exclusively over HTTPS and hosted in France. Technical measures — encryption in transit, anti-automation protections, and restricted access to sensitive files — protect your data from unauthorized access.
